American Medical Association innovation arm Health2047 unveiled its new community-focused and analytics-driven Medicare Advantage plan, which aims to addresses social determinants of health.

Zing Health’s foundation is built on the patient-physician relationship, which it uses to connect customers with health services. With this approach, Zing Health highlights the importance of understanding how social context impacts an individual’s unique barriers to health.

Zing Health’s Medicare Advantage plan can be personalized by each patient to help find community ambassadors, care providers and hospitals. Additionally, each Zing Health customer will have access to a local field care team that collaborates with partner hospitals to coordinate timely procedures and appropriate discharge practices.
